Continuing Healthcare: how a digital solution transformed Cheshire CCGs’ assessment process
NHS Continuing Healthcare is a package of care arranged and funded by the NHS for people who are assessed as having significant ongoing healthcare needs.
But the process, in the past at least, has been largely paper-based with the assessments typically completed manually by health and social care professionals across multiple departments and organisations. The documents supporting a single case can run to hundreds of pages, including several copies and numerous stakeholder touch points.
Five Cheshire and Wirral CCGs, in collaboration with supplier IEG4, have developed the Continuing Healthcare end to end digital solution (CHC2DST) to deliver efficiencies and ultimately improve patient care.

NHS Sunderland CCG sign with IEG4's CHC2DST cloud-based solution
NHS Sunderland CCG responded to the call to action from NHS England last autumn to improve service quality and decision-making efficiency in Continuing Healthcare Assessment. Sunderland CCG has selected IEG4’s CHC assessment process automation tool, CHC2DST, to support the transformation work they are currently undertaking.
